Phenobarbital (Luminal) is a medium to long-acting anti-convulsant barbiturate with sedative properties. Flagyl is metronidazole and when it is combined with Phenobarb, the plasma concentration and therefore the pharmacological effects of Flagyl will be reduced. Significantly increased clearance and shortened half-life of metronidazole will be demonstrated if these 2 drugs are combined. So the metronidazole (Flagyl) dosage may have to be doubled or even tripled to achieve the levels of the drug needed. This is due to hepatic enzyme induction caused by Phenobarbital.
Side Note: It would be helpful for you to have a P450 CYP chart handy when faced with questions like these. Most of phenobarbital’s interactions have to do with induction of certain hepatic enzymes.
